I also have the super basic projectile spell just to try and get the altar working to some degree (projectile + force DMG and nothing else).
A few things to check;
make sure you're using the exact same material through out the base blocks and caps of the altar. If you're using sustone caps, make sure the 'cap' block diectly underneath the altar block is also sunstone and that all other caps are also sunstone. You can't mix and match these, most people miss the extra fifth 'cap' block under the altar block.
Regardless of what the altar block looks like, as long as the block is there then it's working. I've had mine be litterally invisible before but worked perfectly fine.
Make sure the stairs around the top arn't up side down or facing the wrong way, that happenes a lot too. Flat side down, half part up, for the top stairs. Remember, they are not ment to over hang the walls of the altar. The altar is ment to have an arch shape on its inside, i.e. make sure you didn't miss the upside down stairs that form this arch shape.
The altar is 5*5, recount how big it is you may have made it slightly to big. as long as the walls are 3 apart, any extra blocks that extend from the altar are ignored.
there can't be ANY blocks inside of the altars arch, not even torches nor mage torches.
make sure you didn't try and skimp out on any blocks, the base of the altar has to be a fully filled 5*5 base of the same material.

I recommend using witchwood as the base material, in survival, it's easy to get and seems like the least likely to bug out. That and it just looks nice. I've also noticed that in some (old) modpacks, using coal caps wont work.
